Client Rights Notice

Your rights as a counseling client in Texas

You may request a copy of your counseling records at any time. Email dr.stephanieduer@gmail.com or call (936) 296-3696 with your name, date of birth, the records you want, and paper or electronic copies. Electronic records are provided within 15 business days of a written request.

Licensing board

Dr. Stephanie Duer is a Licensed Professional Counselor-Supervisor (LPC-S), Texas license no. 63967. The Texas Behavioral Health Executive Council investigates professional misconduct by counselors and other mental health professionals, and will explain how to file a complaint.

File a complaint

You may also file a consumer protection complaint with the Office of the Texas Attorney General, Consumer Protection Division, for concerns about privacy, access to records, billing or other health information issues.
